PPTC Transfer 2021

Parents / Carers wishing to apply for their child to sit the entrance assessment should download the registration pack below and return the completed registration form and accompanying information by post before 2pm on Friday 16th October 2020.

To be completed and returned to an Assessment Centre by Friday 16th October 2020. (2.00pm)

To assist in the completion of the Entrance Assessment Registration Form and other documents.  It also contains a list of all 33 PPTC schools.

To be completed if you are claiming Access Arrangements for your child (see Guidance Notes).

To be completed by your child's General Practitioner or Medical Consultant in support of a claim for Access Arrangements in relation to a medical condition.

To be completed by a suitably qualified person in support of a claim for Access Arrangements in relation to your child's special educational needs.
